AFSC COMMUNITY AFFILIATIONS
To increase our impact in Orange County we have formed lasting collaborative relationships with a broad range of key healthcare, academic, community-based, and public organizations that share our common vision of supporting families affected by Alzheimer's disease or another dementia.
We are proud to be affiliated with the Neurosciences Institute at Hoag Memorial Hospital Presbyterian (http://www.Hoag.org), and are engaged in joint efforts to improve diagnosis as well as hospital and post-discharge care of patients with dementia. Together with Hoag we created a state-of-the-art facility, where individuals from the earliest to most advances stages of dementia receive compassionate care in a secure environment, and families can access the support and education to help them cope with everyday challenges. Similarly, through our relationship with the UCI Institute of Memory Impairments and Neurological Disorders, we provide a link to critical community-based services that families need.
Deeply embedded in the community, we also achieve our mission by collaborating with the Orange County Alzheimer's Association, Orange Caregiver Resource Center, Orange County Office on Aging, CalOptima, Orange County Transportation Authority and numerous other agencies as well local churches and senior centers in Costa Mesa, Fountain Valley, Huntington Beach, and Newport Beach. To increase our impact, we participate as members and leaders in the Orange County Adult Day Services Coalition, Orange County Aging Service Collaborative, California Association of Adult Day Services, and California Association of Nonprofits.
We are currently collaborating with the Hoag Neurosciences Institute on the new Orange County Vital Aging Program (OCVAP), funded by the UniHealth Foundation and led by Dr. William Rodman Shankle, Program Director, Memory and Cognitive Disorders Program, Hoag Neurosciences Institute. The OCVAP is a community-wide effort to raise knowledge about Alzheimer's disease and related disorders, encourage risk factor management, facilitate early intervention against cognitive impairment, and improve standards of care. Mild cognitive impairment is now known to greatly increase the risk of developing dementia due to Alzheimer's disease or another dementia. OCVAP offers online self-assessment tools, free in-person memory screenings, resources to identify, track, and reduce risk factors and free public education seminars. According to Dr. Shankle, "The OC Vital Aging program is designed to help change the way we deal with our cognitive health. We manage diseases, such as high cholesterol, diabetes and hypertension, through early intervention and monitoring. Why not for our cognitive health? Given the currently available knowledge on risk factors, preventive strategies and treatment for Alzheimer's disease and related disorders, we have a good chance to delay the onset and progression of these disorders. This knowledge needs to be delivered to public and healthcare providers in our community: your brain is your most valuable asset and it's time to give it the same level of attention that the rest of your body gets."
