- Visit Our Center - We invite you to discover our cheerful, serene, and warm environment.
» take a virtual tour of the center right now - Provide Health Records - We'll need a current assessment of your loved one's physical health by his or her physician in order to determine appropriateness of our services.
» download a copy of the Health Record for the physician to complete - Schedule a Home Visit - One of our professional social workers will visit your home to gather a thorough history, determine care needs, and provide resources.
Schedule the First Day - If our services are appropriate for your loved one, your social worker will schedule the first day of attendance. During the first three days of attendance, our team of professionals will assess your loved one and develop an individualized plan of care.
