Day Services
A full day of stimulating, therapeutic activities and services are available to people in the earliest to the most advanced stages of dementia. Activities and services are tailored to each participant's cognitive and physical abilities, and available in Spanish and Vietnamese as well as English.
Health Care Coordination
Individualized Assessment & Care Planning - Members of our friendly and professional care team meet with you and your loved one to understand and assess your needs. Together, they develop an individualized care plan for each person attending the center.
Medical Oversight - Our medical director and pharmacist are experts in geriatric care. They work with our care team to monitor each participant's health and medication regimen. When these geriatricians identify medication interactions or other concerns, our nurses and social workers coordinate improvements in care with you and your loved one's physician or other health care professionals.
Service Coordination - Whether you're looking for legal and financial advice, companion care, or meal delivery, our social workers can link you with community resources to assist you. Your loved one's social worker will help set up and coordinate the variety of support services you'll need across the course of dementia.
Physical and Mental Well-Being
Health Monitoring - Under the direction of the participant's primary care physician, our nurses monitor health conditions of concern, such as hypertension or diabetes, and administer daily medications. By catching and addressing changes early, our nurses prevent health care crises.
Personal Care - Our dementia care aides assist each participant daily, as needed, in accomplishing everyday self-care tasks, such as eating or using the restroom. Incontinence care is available for those participants in the most advanced stages of dementia.
Physical/Occupational Therapy Exercises - Daily maintenance exercises help each participant maintain strength, balance, and coordination. Provided under the direction of licensed physical and occupational therapists, these exercises help participants maintain everyday living skills for as long as possible.
Balance & Mobility - Individuals who are a high risk for falling participate in twice weekly balance and mobility classes. Participants complete a specialized set of exercises developed by the Falls Prevention Center of Excellence.
Home Safety - If your loved one is living alone, prone to falls, or at risk for wandering, our social workers can help you create a safe living environment. You will receive recommendations and resources for making simple, yet possibly life-saving, modifications, such as removing throw rugs or errant electrical cords to reduce the risk for falls.
Individual & Group Counseling - Many participants need someone to talk to about the changes they are experiencing, and help with grieving their losses. One-on-one time with a social worker, or in a therapeutic group with peers, can give a participant the opportunity to share feelings and receive encouragement in coping with day-to-day challenges.
Cognitive Fitness Programs
[m]power™ - A memory stimulation program that is fun and easy to use. Participants receive a 20-minute session that includes enjoyable yet rigorous cognitive exercises via a touch-screen computer.
Cognitively Challenging Games - Our dementia activity specialists have adapted popular games such as Scattegories for players with early dementia. Team games give our early stage participants a fun way to exercise the memory, language, and other skills they want to maintain as long as possible.
Brain Exercises - On a daily basis, all participants exercise the brain through current events discussions, reminiscence, trivia, word games, puzzles, and bingo.
Recreational and Therapeutic Activities
Games - Horseshoes, shuffleboard, putting contest, bingo, the giant dice game, and more keep people physically active and socially engaged with others.
Community Walks & Excursions - Participants can enjoy a walk through our beautiful residential area. Off-site excursions to area museums, restaurants, and community events are also regularly scheduled.
Creative Arts - Therapeutic art and music activities provide each participant with an opportunity for creativity, self-expression, and socialization. And, Dr. Nick, our lovable French bulldog, is everyone's best friend.
Nutrition Services
Daily Meals - A light breakfast, nutritious lunch, and a snack are served each day. Menus conform to a liberal diabetic diet, and special needs can be accommodated. Click here to view this month's menu.
Nutritional Guidance - With support of a skilled nutritionist, our nurses and dementia care aides ensure that your loved one's special dietary needs are met at the center. Additionally, our nutritionist may recommend ways to adjust or improve your loved one's diet at home to meet nutritional goals such as weight gain or loss.
Transportation
Our care managers facilitate transportation to and from the center.
